NHS weight-loss injections

Tirzepatide (Mounjaro®) is a weekly injection that can support weight loss by reducing hunger and helping people feel fuller. On the NHS it must be provided with structured dietary, activity and behaviour-change support.

Current published Year 2 cohort

Adults with a BMI between 35 and 39.9 and at least four qualifying weight-related conditions.

Access is introduced in stages. Meeting the published criteria does not guarantee immediate treatment.

The five qualifying conditions

  • type 2 diabetes
  • high blood pressure
  • atherosclerotic cardiovascular disease
  • obstructive sleep apnoea
  • dyslipidaemia (abnormal blood fats)

BMI thresholds are 2.5 units lower for people from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds. Eligibility and all diagnoses must be confirmed clinically.

Please do not contact the practice solely to request Mounjaro. NHS Greater Manchester is identifying and inviting eligible patients as services become available. The practice will contact you if you are identified for assessment.

Important information about injections

These medicines can be helpful for some people, but they are not suitable for everyone and always require a proper clinical assessment.

How does the staged NHS rollout work?

NHS Greater Manchester is introducing tirzepatide gradually so that people at the greatest clinical risk are supported first. Different areas may deliver treatment through GP practices, GP federations or specialist weight-management services.

The current Year 2 criteria form part of a phased programme, so treatment may not be available immediately even when the published BMI and health-condition criteria appear to be met.

What support comes with the medicine?

Tirzepatide must be accompanied by “wraparound care”, including support with nutrition, physical activity and long-term behaviour change. Patients must agree to take part in this support.

NHS Greater Manchester currently commissions weight-management treatment for a time-limited period of up to two years. Treatment is reviewed for benefit, safety and suitability.

What if I obtain injections privately?

Use only a qualified prescriber and a registered pharmacy. Do not buy injections from social media, beauty salons or unregulated websites.

Tell the practice and any hospital or surgical team that you use a privately prescribed weight-loss medicine. It may affect other medicines, diabetes treatment, contraception, pregnancy planning and arrangements for surgery or sedation.

If you use an oral contraceptive pill, ask your prescriber or pharmacist for specific contraceptive advice when starting tirzepatide or increasing the dose.

When should I seek urgent medical advice?

Seek urgent medical advice for severe and persistent abdominal pain, particularly if it spreads to your back or is accompanied by nausea or vomiting. This can be a sign of pancreatitis.

For urgent advice use NHS 111. Call 999 for a life-threatening emergency.

Other NHS support

Weight-loss medicine is only one possible option. These services may be more appropriate depending on your health and goals.

BMI is a useful starting point, but it does not tell the whole story. Waist measurement, medical conditions, medicines, mobility, ethnicity and personal circumstances also matter.
